“Doctor, I have this tremor in my hands for a long time, will I have Parkinson’s disease? My father died with this problem.” Tremors cause the circumscribed fear in the question, it is understandable. However, Parkinson’s disease is not the most common cause of a trembling hand, nor is it necessary to have a trembling limb to define this condition.

The presence of tremor can be a symptom of the disease, but not a risk factor. However, if it exists, it must meet certain characteristics, as well as be accompanied by other clinical manifestations, such as muscle stiffness and poor movement. It is very rare for Parkinson’s disease to emerge clinically as a single symptom alone.

“No, you don’t have Parkinson’s disease. In fact, there’s no sign that you will.” These few words were what the woman wanted to hear. Relieved and considering herself healthy, she blamed the anxiety caused by everyday issues. She opportunely complained of an aggravating factor, she did not sleep well. Her husband woke her up, he maintained a restless sleep, moved in his sleep, even kicked her.

“No, you don’t have Parkinson’s disease. In fact, there’s no sign that you will, but your husband’s luck will be different.” This second version of the phrase was never said, but it is accurate. Incidentally, the woman very well described to her husband a likely common early sign of brain degeneration which, later on, greatly increases the chances of suffering Parkinson’s disease in its full aspect.

The husband should be moving according to what he dreamed of. If this hypothesis is correct, he had destroyed a group of brain cells responsible for “deactivating” the muscles during REM sleep. This is the deepest phase of falling asleep, the stage in which the eyes move and dreams are wilder. Without muscle “shutdown”, the individual moves in response to dream fantasies. This is REM sleep behavior disorder, one of the possible causes of restless sleep.

Pain in the shoulder blades, depression, anxiety and sudden drops in blood pressure are other clinical events that, if added together, also predict Parkinson’s disease. But there are still two early symptoms, which are very special, as they indicate that the disease does not always start in the brain. These are loss of smell and constipation.

In some cases, not a few, the first degenerative movements injure the cells responsible for olfaction, or the neurons located in the intestine, those that guide the movements of this organ. The disease started a little beyond the nose or in the intestine, ascends to the brain, having a nerve as a guide. This way of putting it forward points out that some people get Parkinson’s disease as a result of something inhaled or eaten.

Static data also reinforce this thesis. The number of people with Parkinson’s disease has more than doubled over the past 30 years and, if nothing changes, will double again by 2040. Population aging alone cannot explain this expansion. Several genetic causes have been identified, but the vast majority of people with this condition do not carry any of these mutations. Brain injuries caused by trauma are also triggers, but in very few affected. Some molecules may be responsible for these data, probably invisible agents that reach mouths or nostrils.

An article published this year in the important journal Jama (Journal of the American Medical Association) demonstrates that years of consumption of water contaminated with trichlorethylene (TCE), a simple molecule of six atoms, increases the risk of suffering from Parkinson’s disease by 70%. Despite being a carcinogen debunked for years, global consumption of TCE continues to increase.

This substance was once the predominant solvent in dry cleaning. Currently, the main uses of TCE are in steam degreasing and in the production of hydrofluorocarbon-based refrigerant gases. Between 9% and 34% of the US water supply has measurable amounts of TCE.

There is no therapeutic intervention capable of curing or reducing the progression of Parkinson’s disease. The recognition of its causes is essential for us to build adequate forms of prevention. TCE is only the first water contaminating molecule to have an association with Parkinson’s disease identified in a large population study.
