The certificate must contain the following information: full name of the participant; diagnosis with description of the condition; code corresponding to the International Classification of Disease (ICD 10); signature and identification of the competent professional, with respective registration with the Regional Council of Medicine (CRM), the Ministry of Health (RMS) or a competent body; and date of service.
